Thank you for your interest in a referral for an adult ADHD assessment. Please read the following three step process carefully.
Due to huge demand, the NHS pathway for Adult ADHD in Derbyshire is currently closed. You therefore have the option of going through the Right to Choose pathway which means you can choose to be referred to a private provider and this will be funded by the NHS.
All Right to Choose providers are classed as private providers.

It is vital that, when selecting a Right to Choose provider, you ensure that:
We appreciate that you will be keen to be assessed as soon as possible, however, please do not choose a provider simply because it has the shortest waiting time. Do check that it is the right service for you.
To proceed with your ADHD referral, please complete the following documentation and return all of these to us:
We can only accept your request for a referral for assessment once all of the above five pieces of documentation have been received.
Please note that if we need to send you reminders for any outstanding information or documents, we will hold your referral for you for a period of 21 days, to allow you time to provide the required information. If you still do not respond to our reminders, we will assume you do not wish to be referred and we will close your request. Doing this allows our admin staff the time to help other patients.
There is no requirement to discuss your ADHD concerns with a doctor beforehand as we will review your submission.
If you are interested in an ADHD/Autism/ASD assessment for a child, please see our mental health page or click the link below. Parents should contact their child's school in the first instance, as school can make referrals to the local mental health service. Comments that GPs must make such referrals are factually incorrect.
ADHD: child referral School pageOnce we have received all five pieces of information, a doctor will review your documentation. If you meet the criteria, the admin team will make the referral to your chosen provider, sending off all the required documents. After this, your chosen provider will be in touch. If you don't meet the criteria for referral, you will be informed and signposted to general support.
Mental health supportOnce your referral has been sent to your provider, the surgery will no longer be involved in the process and all queries will need to be directed to your provider.
