Short survey by our Patient Reference Group

We are a newly formed group of patients working with the practice to look at what is good about Ivy Grove and what could be improved. We would be very grateful if you could complete this questionnaire so that we can learn more about other patients.
The practice runs extended hours (from 7.30am Tuesday to Friday and until 7.30pm on Monday)
Did you know you can book appointments in advance? You do not have to book on the same day
Did you know that there are other options for healthcare support (especially after the GP surgery has closed)?
Walk in centres in Ilkeston and Derby (8.00am-8.00pm Monday to Sunday)
Out of Hours GP Service (if you need to see a GP after the surgery has closed)
NHS Direct (advice if you have concerns about what you should do)
Local Pharmacy (you can ask about medication and over the counter options)
If you needed healthcare when the GP surgery is not open do you think you would access one of the above services or just wait until the surgery opened again either the next day/Monday morning?
